I consent to the use of sunscreen, bug spray and topical ointments on my child while at school. For medical or other reasons, please do not apply any topical ointments to my child's body.

I will provide sunscreen with my child's first and last names written on the bottle on the first day of school and I will make every effort to apply sunscreen to my child before they arrive at school every day.
